Letter from the Executive Director and Board Chair
Dear HVO Community,
2025 was a challenging year for global health. With critical funding cuts, wars, disease outbreaks, climate change, and the continued health workforce shortage, the need to invest in healthcare education and training has never been greater. Throughout the year, HVO volunteers and partners gave us cause for hope by showing us how much can be accomplished through collaboration, dedication, and respect. Together, we maintained over one hundred projects and opened ten more, providing training to thousands of health workers around the world, many of whom have already trained countless others. Every volunteer visit, virtual lecture, mentorship session, and training program represented another step toward a future where local health workers have the knowledge, confidence, and support they need to provide excellent care in their own communities.
Lasting change does not happen overnight. It is built through relationships, trust, and a shared commitment. At Health Volunteers Overseas, we have long understood that sustainable impact on global health comes from investing time, expertise, and resources in local health professionals who will continue serving their communities for years to come. Throughout 2025, our volunteers and partners worked together to share knowledge, mentor future leaders, and expand access to high-quality care. In the stories below, you will see examples of partnerships grounded in mutual respect and a recognition that local health care workers are the driving force behind stronger health systems. In Nepal, Dr. Melanie Thomas and Dr. Soniya Dulal are transforming cancer care around the country. In Paraguay, Dr. Rómulo Silva and Dr. Tomás Gonzalez are making knee replacement surgeries possible for hospital patients who did not previously have access. And in Cambodia, Dr. Luke Carter-Brzezinski and Chantha Hou have helped to develop the best hematology lab in the country, saving the lives of patients with cancers and blood disorders through faster and more accurate diagnoses.

Stronger Together: Building Oncology Care in Nepal
In Nepal, a severe shortage of oncologists limits access to life-saving cancer care. There is roughly one medical oncologist for every 750,000 people, compared to one for every 18,750 in the United States. Dr. Soniya Dulal, a medical oncologist at B.P. Koirala Institute of Health Sciences (BPKIHS) and an HVO on-site coordinator, is working hard to change this. With support from Dr. Melanie Thomas, HVO project director, Dr. Dulal was successful in establishing a new oncology residency track at her hospital. While this was a huge step, it still did not feel like enough. In the fall of 2024, Dr. Dulal and Dr. Thomas organized the first Oncology Essentials workshop at BPKIHS for thirty-four internal medicine residents from around the country to learn the basics of triaging and treating oncology patients. The workshop was such a success that they held a second one in the fall of 2025, reaching even more providers.

Building Surgical Bridges
Dr. Rómulo Silva, an orthopaedic surgeon in Portugal, was one of the first volunteers to serve at HVO’s new partner site at Hospital de Clinicas in Asunción, Paraguay when it opened in 2025. He formed a particular connection with Dr. Tomás Gonzalez, a local knee surgeon. “It felt like we had known each other for a long, long time,” said Dr. Silva. “We learned together. We connected as people.” Dr. Silva also worked with the residents, accompanying them on rounds and discussing cases. He invited one resident to visit his home institution in Portugal, and the resident stayed and learned for a month. “This is the most impact we can have,” said Dr. Silva. “He sees how we operate, and not just on knee surgery. What he will get from this experience will be much more than the one week I was able to spend there.”
Speaking about what the experience has meant to him, Dr. Silva said, “We never know what will come our way, as COVID showed us. I think expanding your vision is very important. When you go places where people have access to better technology and resources, you will get to see people doing things you rarely have a chance to do. But it’s also very important to see people managing without access to the same resources. In Portugal, in the US, as doctors we always face inconveniences and things we don’t expect. We need to be ready for challenges. Having these volunteer experiences, learning from other people, this is the best way. Teaching and learning, they are connected. It is difficult to teach without also learning.”
Friendship, Trust, and Lifesaving Care
Ms. Chantha Hou, head of the hematology and flow cytometry laboratory at Calmette Hospital in Phnom Penh, Cambodia, has been working with HVO volunteer Dr. Luke Carter-Brzezinski since early 2024. During his three-month visit, Dr. Carter-Brzezinski focused his teaching on blood cancers and how to analyze bone marrow and blood. Since then, Dr. Carter-Brzezinski and other HVO volunteers have formed a multidisciplinary case conference team, meeting every month for the last two years with Ms. Hou, hematologists, and histopathologists at Calmette Hospital. No longer feeling isolated is one of the biggest changes Ms. Hou feels has come from HVO partnership: “Before, I felt like I was alone. Now, when I have difficult cases, I can discuss with HVO colleagues. I feel confident. I have a friend.”
Ms. Hou says that Calmette Hospital has the best hematology lab in Cambodia, in part thanks to education provided by HVO volunteers. “I hope HVO will continue to build education and support me in developing cytogenetics capabilities at my lab in the future,” said Ms. Hou. “Thanks to my own training, I now feel confident training other people. I do not feel alone.”

Ameen Barghi, MD | Orthopaedic Traveling Fellow
This experience expanded my understanding of global health by showing how high-quality care can still be delivered in resource-limited settings through creativity, teamwork, and adaptability. I built lasting connections with colleagues across the world, and it has motivated me to remain involved in global surgical education and capacity building throughout my career. This experience reinforced my commitment to mentorship and strengthened my belief that collaborative learning benefits both the learner and the teacher. After one case, a junior resident pulled me aside and said, “I’ve always been nervous to place a distal femur plate, but after walking through it step-by-step with you, I feel like I can do it.” That moment reflected a real shift in confidence and understanding – something that can ripple forward into better patient care. Another highlight was during a teaching session where we discussed fracture classification and fixation principles using only X-rays and whiteboard sketches. Despite the limited resources, the room was full of engagement and thoughtful questions. It was a reminder that education doesn’t depend on high-tech equipment – it depends on presence, patience, and partnership. These experiences made me feel like I was helping empower local trainees not just for one case or one week, but in a way that might influence how they approach surgery and teaching going forward.
Paul Nguyen, MD | SEA-HVO Fellow
From September 20 to October 19, 2025, I traveled to Kabale, Uganda to serve as a volunteer anesthesia educator at Kabale School of Medicine (KABSOM). Over four weeks, I observed and assisted in clinical care across the maternity and medicine wards, ICU, and operating rooms. I helped students develop skills in airway management and spinal anesthesia while emphasizing safe vitals monitoring. I am currently collaborating with KABSOM students to develop a Book of Standard Dilutions, a practical, locally adapted reference that will support their clinical preparation long after volunteers have returned home. What we built in that month motivates me still. It was one of the most meaningful experiences of my career, and I hope to return. I had the privilege of supporting trainees as they intubated for the first time and of watching their confidence grow as they safely placed spinals for C-sections. Beyond the technical skills, what stood out the most was their enthusiasm and sense of responsibility to their patients and communities. Many spoke about their hopes to return to their hometowns to work as an anesthesia provider. Sharing in those moments of learning and reflection was incredibly rewarding.
Kenneth Nyombi, MD | Orthopaedic Scholar (Uganda)
On April 1st, 2025, I left Uganda for Texas to do a clinical rotation at Texas Back Institute, the institution of my mentor, Dr. Isador Lieberman. My interest is in spine deformity surgery. Working with and watching Dr. Lieberman correct several deformities in the days I spent with him in the OR was beyond an honor. I was particularly keen to learn how spine osteotomies are done for the shortening the posterior column during rigid deformity correction. I was also exposed to the Anterior Lumbar Interbody Fusion (ALIF) technique, which we don’t often do back home.
I followed Dr. Lieberman to every patient he saw. I observed the way he approached his consultation, how he interrogated his patients for issues of interest, and how he sought consent for surgery. I remember his rule was not to convince the patient to have the procedure, but rather to give them all the information to make the decision. I adopted this kind of humility and patient approach, especially with regard to consent for surgery. I had the opportunity to observe one of the world’s best deformity surgeons in the operating room. I appreciated Dr. Lieberman’s soft tissue handling technique, his placement of posterior column osteotomies, and his logical approach to deformity correction. Although I have resource limitations, I have incorporated spinal osteotomy in my practice. This visit was my first experience seeing robotic screw placement. The speed, accuracy, ease, and reproducibility are all unmatched. Although I do not expect to have robot in my Ugandan practice anytime soon, I was very thankful for the exposure and experience. I thank the HVO Orthopaedic Scholarship for funding my entire trip to the Texas Back Institute. It had always been my dream to visit TBI, and indeed you made my dream come true.
Rojna Rai, MD | HVO-Wyss Scholar (Bhutan)
I participated in the 22nd World Congress in Fetal Medicine organized by the Fetal Medicine Foundation (UK), from June 29-July 3, 2025, in Prague, Czech Republic. I submitted one of my research projects on twin pregnancies in my hospital, which was accepted for a poster presentation. It is the dream of every fetal medicine specialist to attend the FMF World Congress in Fetal Medicine. The scientific sessions of the congress were packed with lectures, research presentations, panel discussions and guideline summaries. During the breaks, the conference organizers had set up stalls featuring world-leading companies who manufacture the latest technologies used in fetal medicine. It was an eye-opener to witness these technological marvels, and I hope that I can bring some of them to Bhutan. I met Professor Kypros Nicolaides, the founding chairperson of the Fetal Medicine Foundation. He is one of the pioneers of fetal medicine, and his discoveries have revolutionized the field. He offered to help with training and guidance through hand-holding of our nascent fetal medicine program. I also met Dr. Suresh Seshadri from Mediscan Systems, a leading fetal medicine center based in Chennai, India. He extended an invitation to their center so I could learn from them.
I also spent two weeks doing a clinical attachment at Charite Medical University, one of the largest medical universities in Germany. The consultants were very knowledgeable, and some of them were pioneers in the field of fetal medicine. I observed fetal intervention procedures such as amniocentesis and laser photocoagulation for twin-to-twin transfusion syndrome, apart from observing fetal ultrasonography. I hope to formally establish institutional linkages between them and our department. A clinical exchange program, their remote guidance in our local guideline development, collaborative research, participation in conferences and seminars, etc., are various ways in which we can expand our collaboration in the future.
Once home, I have planned to conduct a series of lectures/updates for our faculty and residents on the fetal medicine topics that were covered at the conference. I am also planning to conduct a workshop on fetal ultrasound for obstetricians, residents, ultrasonographers, radiologists etc., incorporating the latest techniques and guidelines which I learned at the conference.
Getting the opportunity to present a poster at this prestigious conference certainly adds a lot of value on my research portfolio and resume. Moreover, the knowledge and experience I have gained from this conference is immense. It will not only benefit me – by sharing the knowledge and the skills I have acquired, it will help my entire department, hospital, and country as a whole.
